What is down regulation in IVF?

What does down-regulation mean and why it is needed?

When growing egg during an IVF cycle, one makes use of 2 processes – one is superovulation, while the other is down-regulation. Think of these as the 2 levers which do allow us to precisely control the egg production.

Downregulation happens to be a process of reducing the sensitivity of a cell to a specific hormone or chemical. This is no doubt achieved by reducing the number of receptors on the surface of the cell. Receptors are how the cell can rather process and receive signals that come from outside of the cell. When an external substance is bound to a receptor, it does fit like a key in a lock and the receptor becomes full. Once this binding occurs, the substance can then also provide directions to the cell on what needs to be done (release a substance, etc.). The more cell the receptors are available, the greater the response to the external substance. 

With down-regulation, the cell’s receptors are rather overexposed to an external substance (often a hormone) and this exposure does cause the body to decrease the number of receptors on the cell’s surface, which in turn has the effect of reducing the cell’s sensitivity. During fertility treatments, several medications are made use of for down-regulating various aspects of the female reproductive system. 

During an in vitro fertilization (IVF) cycle, gonadotropin-releasing hormone (GnRH) agonists such as Lupron, Synarel, Suprecur, or Buserelin are made use of to prevent premature ovulation.
